Non-Emergency Medical Transportation (NEMT) Annual Training Policy
Purpose: This policy ensures that all Mavride LLC drivers participate in annual training to maintain a high standard of safety, professionalism, and compliance with industry regulations.
Scope: This policy applies to all drivers employed by or contracted with Mavride LLC.
Policy Statement: Mavride LLC is dedicated to providing annual training for all NEMT drivers to ensure continuous improvement, regulatory compliance, and enhanced customer service.
Annual Training Requirements:
- Annual Defensive Driving Course: All drivers must complete an annual defensive driving course to reinforce safe driving habits and mitigate risks.
- CPR and First Aid Recertification: Drivers must maintain current CPR and First Aid certification with annual recertification.
- HIPAA and Confidentiality Refresher: Annual training on HIPAA compliance and patient confidentiality to protect client information.
- Customer Service and Sensitivity Training: Annual refresher courses on professional conduct, dealing with diverse populations, and providing compassionate service.
- Safety and Emergency Protocols: Annual review of accident/incident procedures, emergency evacuation, and passenger assistance techniques.
- Vehicle Safety and Maintenance Updates: Annual training on vehicle inspection, maintenance procedures, and cleanliness standards.
- Regulatory Updates: Annual briefing on state and federal transportation law updates and changes in industry regulations.
Training Delivery:
- Training will be provided through online modules, in-person sessions, and hands-on demonstrations.
- Attendance and participation in all annual training sessions are mandatory.
Documentation:
- All annual training records, completion certificates, and acknowledgments will be maintained in each driver’s personnel file.
Non-Compliance: Failure to complete annual training will result in suspension from driving duties until all required training is fulfilled.
Policy Review: This policy will be reviewed and updated annually to reflect best practices and regulatory requirements.
Acknowledgment: All drivers must sign an acknowledgment form confirming they have read, understood, and agreed to adhere to this annual training policy.

Non-Emergency Medical Transportation (NEMT) Driver Training Policy
Purpose: The purpose of this policy is to ensure that all Mavride LLC drivers are properly trained to provide safe, reliable, and professional transportation services to our clients, with a focus on compliance, safety, and customer service.
Scope: This policy applies to all drivers employed by or contracted with MavRide LLC.
Policy Statement: MavRide LLC is committed to ensuring that all NEMT drivers receive thorough training before transporting clients and continue ongoing education throughout their employment.
Training Requirements:
- Initial Training: All drivers must complete initial training before transporting passengers. This includes:
- Defensive driving techniques and road safety protocols
- Passenger assistance procedures (wheelchair, stretcher, and ambulatory clients)
- HIPAA compliance and patient confidentiality
- CPR and First Aid certification
- Vehicle inspection, maintenance protocols, and cleanliness standards
- Accident/incident reporting procedures
- Professional conduct and customer service etiquette
- Background Checks and Screenings:
- Criminal background check
- Drug and alcohol screening
- DMV driving record check
- Ongoing Training:
- Annual recertification in CPR/First Aid
- Quarterly safety and compliance refreshers
- Annual defensive driving course
- Ongoing updates on state and federal transportation regulations
- Documentation:
- Training completion certificates and documentation must be kept on file for each driver
- Drivers are to acknowledge receipt and understanding of this policy
Non-Compliance: Failure to comply with these training requirements will result in a suspension of driving duties until all training and certification are complete.
Policy Review: This policy will be reviewed annually and updated as necessary to comply with regulatory changes and industry best practices.

Mavride LLC
Non-Emergency Medical Transportation (NEMT) Drug Screening Policy
Purpose: The purpose of this policy is to ensure that all Mavride LLC drivers comply with drug and alcohol screening requirements to promote a safe and drug-free workplace, ensuring the safety of clients and employees.
Scope: This policy applies to all drivers employed by or contracted with Mavride LLC.
Policy Statement: Mavride LLC is committed to maintaining a drug-free workplace and requires all drivers to undergo drug and alcohol testing in accordance with federal, state, and company guidelines.
Drug Screening Requirements:
- Pre-Employment Testing: All prospective drivers must pass a drug screening before employment or contract activation.
- Random Testing: All drivers are subject to random drug and alcohol testing throughout the year.
- Post-Accident Testing: Drug and alcohol testing will be required immediately following any accident or incident involving a vehicle or client, in accordance with regulatory guidelines.
- Reasonable Suspicion Testing: Drivers may be required to undergo testing if there is reasonable suspicion of drug or alcohol use based on observed behavior, appearance, or performance.
- Return-to-Duty Testing: Drivers returning to duty after a positive test result or policy violation must complete a return-to-duty drug test and be cleared by management.
- Follow-Up Testing: Follow-up testing may be required as part of a rehabilitation or return-to-work program.
Prohibited Substances:
- Marijuana (including medical marijuana)
- Cocaine
- Opiates
- Amphetamines
- Phencyclidine (PCP)
- Alcohol (within prohibited limits)
Consequences of Non-Compliance:
- A positive test result or refusal to submit to testing will result in immediate suspension and possible termination.
- Drivers may be referred for counseling or rehabilitation services at management’s discretion.
Confidentiality: All drug and alcohol test results will be kept confidential and shared only with authorized personnel.
Policy Review: This policy will be reviewed annually and updated to reflect legal requirements and industry best practices.
Acknowledgment: All drivers must sign an acknowledgment form confirming they have read, understood, and agreed to comply with this drug screening policy.

Mavride LLC
Independent Contractor Agreement
This Independent Contractor Agreement (“Agreement”) is made effective as of Date, by and between:
Mavride, LLC, located at 440 Louisiana Str Ste 900, Houston Texas 77002, hereinafter referred to as the “Company”,and
The driver’s name, hereinafter referred to as the “Contractor”.
1. Services to be provided and job description:
- The Contractor agrees to provide the following services to the Company:
- Transport patients and clients safely and professionally using your own/company vehicles.
- The Contractor agrees to the following Job description:
- Communicate effectively with dispatch regarding the schedule and receiving instructions.
- Maintain a professional image and attitude regarding patients, family members, and co-workers.
- Complete daily vehicle pre-shift and post-shift inspections and maintain vehicle cleanliness.
- Assist clients/patients as needed to complete the transfer safely.
- Perform all duties as assigned by the dispatcher.
- Ensure client safety and comfort during transportation.
- Follow all traffic laws and regulations.
- Conduct vehicle inspections and reporting any maintenance or repair needs.
- Adhere to company policies and procedures
2.Term of Agreement:
This Agreement shall commence on Current Date and shall continue until terminated by either party upon written notice.
3.Compensation:
The Company agrees to pay the Contractor bi-weekly, effective after the first three weeks of driving.
4.Independent Contractor Status:
The Contractor acknowledges that they are an independent contractor and not an employee of the Company. The Contractor is responsible for paying all taxes, including self-employment taxes, and obtaining any necessary insurance.
5.Confidentiality:
The Contractor agrees to keep confidential all information related to the member’s personally identifiable information, and the Company’s business, including but not limited to, financial information, and trade secrets.
6.Termination Clause:
Termination by Company: The Company reserves the right to terminate this Agreement immediately and without prior notice if the Contractor fails to comply with the job description, duties, and responsibilities as outlined in this Agreement, or if the Contractor engages in conduct that is detrimental to the Company or its interests.
Termination by Contractor: The Contractor may terminate this Agreement by providing the Company with a written notice at least two (2) weeks before the intended termination date. Such notice must be delivered either via email or letter to the designated representative of the Company.
Final Obligations: Upon termination of this Agreement, the Contractor shall return all Company property and settle any outstanding obligations as per the terms of this Agreement.
7.Governing Law:
This Agreement shall be governed by and construed in accordance with the laws of the State of Texas, United States.

Purpose: The purpose of this policy is to outline the process for investigating and responding to incidents and accidents involving Mavride LLC drivers and vehicles, ensuring the safety of clients and employees, and maintaining compliance with legal and regulatory standards.
Scope: This policy applies to all drivers employed by or contracted with Mavride LLC and covers all accidents, incidents, or near-miss events.
Policy Statement: Mavride LLC is committed to thoroughly investigating all accidents and incidents to determine causes, implement corrective actions, and prevent future occurrences.
Incident/Accident Reporting Requirements:
- Immediate Notification: All incidents or accidents must be reported to Mavride management and dispatch immediately, regardless of severity.
- Medical Attention: Drivers must ensure that any injured parties receive prompt medical attention and that emergency services are contacted if necessary.
- Law Enforcement Notification: Law enforcement must be contacted for all accidents involving injuries, property damage, or as otherwise required by law.
- Documentation: Drivers must complete an incident/accident report form within 24 hours of the event and submit any supporting documentation, including photos and witness statements.
Investigation Process:
- Initial Review: Management will review the driver’s report and gather additional information from dispatch, passengers, and any witnesses.
- Site Inspection: A site visit may be conducted by management or a designated safety officer if necessary.
- Root Cause Analysis: A thorough review of contributing factors (driver behavior, vehicle condition, weather, road conditions) will be conducted to determine root causes.
- Corrective Actions: Corrective measures may include driver retraining, vehicle repairs, disciplinary action, or updates to company policies.
- Reporting and Recordkeeping: All investigation findings and actions taken will be documented and retained in the company’s records.
- Regulatory Reporting: Accidents or incidents that meet reporting criteria will be reported to regulatory bodies as required by law.
Policy Review: This policy will be reviewed annually and updated as needed to reflect best practices and regulatory requirements.
Acknowledgment: All drivers must sign an acknowledgment form confirming they have read, understood, and agreed to comply with this incident/accident investigation policy.

- HIPAA and Confidentiality: Instruction on patient privacy laws and maintaining confidentiality in all client interactions.
- CPR and First Aid: Completion of CPR and First Aid certification, if not already certified.
- Vehicle Operations: Training on vehicle inspections, cleanliness standards, maintenance responsibilities, and fuel efficiency practices.
- Documentation and Reporting: Guidance on completing trip logs, timesheets, and incident reports accurately and timely.
- Technology Training: Orientation on the use of dispatch apps, GPS navigation, and other company technology systems.
Documentation:
- Orientation completion records and signed acknowledgment forms will be kept in each driver’s personnel file.
Non-Compliance: Drivers will not be permitted to transport clients until all orientation components are successfully completed.
Policy Review: This policy will be reviewed annually and updated as needed to reflect best practices and regulatory requirements.

- Punctuality and Reliability: Drivers are expected to be punctual, complete all assigned trips on time, and communicate promptly regarding delays or issues.
- Documentation Accuracy: Trip logs, timesheets, incident reports, and any other documentation must be completed accurately and submitted on time.
- Communication: Drivers must maintain clear and professional communication with dispatch, clients, and Mavride management.
- Professional Conduct: Any behavior deemed unprofessional, disrespectful, or unsafe will result in corrective action.
Performance Evaluation:
- Performance reviews will be completed biannually.
- Client feedback and complaints will be reviewed and addressed.
Corrective Action:
- Drivers not meeting performance standards may receive verbal or written warnings.
- Continued non-compliance may result in suspension or termination.
Policy Review: This policy will be reviewed annually and updated to ensure it aligns with company goals and industry best practices.
